If you have a claim, report it immediately!

Call (800) 453-4715. This number is for the initial reporting of the claim to get the process started. All accounts need to identify themselves as part of the TECIS program.

When calling the 800 number above, be sure to have the name of the employee, social security number, date of hire, etc. See Form 5020 they will need the information on this form taken verbally on the phone. There is also Form #32-10 or DWC Form 1, which you fill out and sign and give to the injured employee to sign.

Once the initial claim has been reported to the above number, the local adjusting office is:

You will be assigned a claim number. All of the necessary follow-up with the hospitals, doctors, and, of course, the employee will be handled by GAB Robins.

You may call GAB Robins if you need further information regarding the claim or if you receive a bill from a doctor or hospital that requires their attention.

Employees should be taken to the appropriate facility by a manager whenever possible.
